This is an agreement for contractual counseling services with Relentless Hope. You are being retained to provide our client with support therapy in relation to the ritual abuse/child trafficking the client has experienced. This benefit cannot be transferred to any other person than the client listed below. You understand that your services will be retained for a set number of sessions and at an agreed upon hourly rate.
You will be required to provide a monthly report on the number of completed sessions for accounting purposes. You understand that if the client isn't attending sessions, you will notify Relentless Hope of that fact so we can evaluate next steps. Relentless Hope understands that client confidentiality is assured and we will not request specifics about the client therapy discussion. This agreement will comply with all HIPAA requirements.
Relentless Hope does not support therapy that is not American Psychological Association (APA) approved. Relentless Hope does not support hypnotherapy as it renders anything the client reports inadmissible in court.
This agreement can be cancelled by either party with a 14 day notice. This agreement is not a guarantee of payment if the client fails to attend a session. Client will be responsible for payment of any missed appointments not in compliance with your cancellation policy.
